The rear locking device popped free and struck the employee's left hand, which was pinned against the upper supporting member. An employee was hooking up a flatbed trailer to a pickup truck. While letting the hitch down onto the receiver ball, the hitch became bound. The employee jostled the hitch pin while continuing to let the jack stand down. The hitch slipped while the employee's fingertip was in the safety pin hole, causing a fingertip amputation.

An employee was checking a power swivel's back-up securing line to see if it was tight enough. As he reached out to slap the line, residual stored energy released, causing the securing line to back spin. The line was attached to a turnbuckle with a safety pin in place. The safety pin spun backwards, catching and lacerating the employee's right inner elbow area and causing potential muscle damage.

An employee was attempting to raise the tongs up off the rig floor with a hoist when a safety pin became hung on the floor grating. The employee had his hand on the tongs while trying to see what they were hung up on. At that time, the tongs came free from the floor grating and caught the employee's left pinky finger between the tong body and mid rail resulting in amputation.
